During the period March 7, 2011 to April 18, 2011, you shipped LeanSpa products to my address at (Personal Information Removed). I left home on March 7, 2011 and went on vacation to the Philippines and stayed there through the month of April 2011 until May 25, 2011.

I was able to take note of the shipments only sometime on April 17, 2011, while abroad. Upon learning about them from the Internet, and for the reason I did not order the products, I emailed a letter (copy enclosed as Enclosure “A” for ready reference) to LeanSpa stating my demand to refund the purported amounts that have been collected. I followed that up with another email dated April 19, 2011, copy also enclosed as Enclosure “B”, wherein I asked you to stop the shipment LeanSpa appears to have again shipped the product under Invoice # 520116-3 on April 18, 2011, a day before the email date, 4/19/11

There were three shipments. The first shipment, consisting of one package, was on March 7, 2011; the second, consisting of two packages, was on March 28, 2011; and the third, consisting of two packages, was on April 18, 2011.There are in all seven (7) bottles that were shipped to my said address – five of them are The PROPER Weight Loss System and two, LeanSpa Cleanse. The shipments appear to have been done by you on the dates, as follows:
1. March 07, 2011-- Shipment of “Order NO. 520116-1a”, consisting of LeanSpa – The PROPER Weight Loss System, Quantity: 1
2. March 28, 2011 -- Shipment of “Order NO. 520116-2”, consisting of LeanSpa – The PROPER Weight Loss System, Quantity: 2
3. March 28, 2011, -- Shipment of “Order NO. 520149-2”, consisting of LeanSpa Cleanse, Quantity: 1
4. April 18, 2011 -- Shipment of “Order NO. 520116-3”, consisting of LeanSpa – The PROPER Weight Loss System, Quantity: 2
5. April 18, 2011, -- Shipment of “Order NO. 520149-3”, consisting of LeanSpa Cleanse, Quantity: 1

From the shipment notifications sent to me online, I noted that the Invoice sheets indicate no price tag for the products described therein. But from my bank statements of account, I noted that LeanSpa charged six times the uniform amount of $79.99 against my bank account purportedly as purchase cost and collected the same on the dates indicated below, as follows:

From the above list, it appears that you have collected from my bank account a total of FOUR HUNDRED SEVENTY-NINE & 94/100 ($479.94) US DOLLARS.

As stated in my email of April 17, 2011 (Enclosure “A”), I demanded, among others, cancelation of the alleged subscription, and the refund to me of the charges collected from my bank account. You responded, and in your email of April 21, 2011 enclosed as Enclosure “C”, you confirmed that my account has been canceled. In addition, you sent me the RMAs, and wrote that once the package is received (by you), as stated in said Enclosure “C”, a credit will be issued to my credit card. Further, on April 23, 2011, in your email to me, enclosed as Enclosure “D”, you offered a quick cancel option, which if I chose I would be able to keep bottles for only 19.41 for each of them. And you will refund me the sum of $60.58 & $60.58 immediate. But because on that date I had no idea, as I was out of the country then, how many bottles were shipped to my address, I was unable to accept or reject the offer.

You sent me two RMA numbers – RMA # 520116-521 for Order number 520116 (LeanSpa with Acai) and RMA # 520149-520 for Order number 520149 (LeanSpa Cleanse). If I got your message right, RMA # 5201160-521 applies to:
1. Shipment of “Order NO. 520116-1a”, March 07, 2011, consisting of LeanSpa – The PROPER Weight Loss System, Quantity: 1
2. Shipment of “Order NO. 520116-2”, March 28, 2011, consisting of LeanSpa – The PROPER Weight Loss System, Quantity: 2
4. Shipment of “Order NO. 520116-3”, April 18, 2011, consisting of LeanSpa – The PROPER Weight Loss System, Quantity: 2
While RMA # 520149-520 applies to:
3. Shipment of “Order NO. 520149-2”, March 28, 2011, consisting of LeanSpa Cleanse, Quantity: 1
5. Shipment of “Order NO. 520149-3”, April 18, 2011, consisting of LeanSpa Cleanse, Quantity: 1

With this understanding, I am returning to you all the seven bottles enumerated above. For convenience, I put together in one package (a box) each and all the individual pack that were originally shipped to my address. I wrote the two RMA numbers -- RMA # 5201160-521 & RMA # 520149-520, on the outside of the box. I also wrote the RMA numbers on the outside of each individual pack as originally mailed to my address.

Please take note that this return took me some time to do for the reason stated in my email dated April 21, 2011 enclosed as Enclosure “E”.

And kindly refund to me by crediting to my bank account the total amount of $479.94 charged by you and collected from my said bank account, as above stated.
