On November 6, 2013 USA Health and Wellness Services, Inc ran a transaction for $321.25 without my authorization. I tried to contact them and was reassured that the money would be reversed back to my account, but would take about 2 weeks. After 2weeks, I tried to contact them, but they were no longer in business.
