Judging Sheet attached-- to be mailed back but send with $12.99 if I want to get the Premium Offer of $2500 Gift Certificate.The returnable Judging sheet, "Official Paperwork" asks questions to be checked: Yes, my name is___, "I testify I am 18 years old-completely eligible in this matter", "I testify I am not an employee of the company and I have never won a major cash award before". Asks to check one option: $100,000 year/30 years or Lump sum certified check $1.980,000.An envelope stamped "URGENT" is included for return of the form and my 12.99 check for "Premium Offer purchase for Delivery."
