It was a questionaire for Kohls. for taking this little quiz a free gift with only $2.95 for s/h on 9/18/13. Then on 10/7/13 they took $97.88 out of my account. The phone #866-216-9336 is not their number, but that of a medical shipping supply company. I want my money back & have no way to contact this company. I received a skin care pack.
