I received a check in the mail for $3,990.00 along with a letter titled "Prize Claim/Final Notice". The winning programs were supposedly held on Feb 25, 2013. I supposedly won in the 2nd category. Ticket serial #408662257, (which I have no idea how I acquired since I never entered anything), drew the lucky winning #40-56-78-81-03. I'm supposedly entitled to the sum of $150,000.00. My claim # is JD/HGB-0202. I'm not supposed to attempt to use the check until I call my Claim agent who is Lloyd Lewis, 1-289-892-3517. I'm being urged to keep this winning confidential until my claim has been processed. There are supposedly Special Conveyance Offices with Lyne Wackley as Registrar. The letter states that the sole purpose of the $3,990 check is for payment of applicable Government taxes that cannot be deducted from the winning amount because of the insurance policy coverage. The tax amount is $1,995 US dollars to be paid directly to the tax agent. Enclosed check is from US Buildings, LLC, Boone, NC 28607. Bank is First-Citizens Bank & Trust Company
