My mother received her May bank statement and noticed an out of sequence check for $447.34. She went to the bank and they printed the check. As stated by many others, it was a typed check made out to YR Benefits. No signature was supposedly required. Bank started an investigation for fraud, however, we also called teh phone number listed - 888-778-0073. They answered, "3rd Party Benefits". When we questioned the transaction, I was told my 84 year old mother authorized them to provide her wiht eye coverage. This was untrue. They claim she will be refunded within 21 days, stating she can cancel her coverage since it wasn't used. I explained she never authorized coverage. They quickly got off the phone.
