COMMENTS ON CHECK " CUSTOMER AUTHORIZATION OBTAINED 04-01-12. I DID NOT AUTHORIZE THIS TRANSACTION. IT ALSO CAUSED MY CHECKING ACCOUNT OVERDRAWN AND MY BANK, PNC BANK CHARGED $36.00 NSF/OD FEE.
