Insurance changed without my knowledge in May 2014~they said it defaulted…I was on MLOA for most of 2013. Back to work Jan.1,2014~March 2014. Dr has me off on a MLOA now. The representative said this change occurred in Feb.2014 when I was back at work. When premiums were then pulled out of my check…now it is May 2014,and just two days ago that my plan changed to this Consumer Insurance($1,500. deductible) Plan I never opted in to. I was paying for my premiums the 11 1/2 months I was out in 2013,with my State Disability income. This expires after a year is up. The Long term insurance company i have with them said I needed to apply for social security,in order to keep my plan in effect.
