Without my permission, without any signed document or authorization, an agent signed me up for a Fidelity Life Association Accidental Death and Dismemberment policy in the amount of $500,000. The monthly fees of $47.85 were automatically debited from my credit card. Admittedly, I was careless about following up on the debits and did not call on it for about 2 years. Since I have an investment account with a company of a similar name, I assumed it was connected to that. When I inquired with my investment advisor, he indicated this charge was not associated with my investment account in any way.

I phoned Fidelity Life Association Customer Service and was told an agent named Vashon Moore set this up. I have been unable to reach him. I was told to send a letter to Fidelity Life to cancel future payments. That does not help me recover the lost 29 payments already deducted.

This is a scam artist who may have obtained my personal information when I applied for health insurance. I don't know but others should be aware of the fraudulent and unethical business practices of this company. I do not want or need any such account for Accidental Life Insurance EVER.
