As per my investigation for my client,we have supporting document copies of course completion documents and transcripts, as we contacted the original training provider with no response after being told to pick up the cards 2/19/2014. It is further evidenced that the trainer received course completion cards from OSHA, per their call to my client and they did not distribute. I am providing the relevant information:
trainer's name, Above Average, Llc
end date of training, October 2013
type of training, OSHA
training location, NYC
actions taken to try to resolve the issue- contact with the agency contact with OSHA dept and complaint to US Attorney General.
There website is down and their phone just rings.
