MY BANK PAID 87.00 FROM MY ACCOUNT WHICH I WAS NOT AWARE OF.WAS TOLD ON THE PH0NE THAT THE MONEY WOULD BE REPLACED IF I WOULD FILL OUT FORM I WAS AT THE BANK AT THE TIME YOUR CUSTOMER SERVICE AGENT SAID SHE WOULD FAX THE FORM FOR ME SIGN. I GAVE HER THE FAX NUMBER AND NOTHING HAS BEEN FAXED. PLEASE CALL ME AT (Personal Information Removed) AND EXPLAIN WHY NOTHING WAS DONE. LONNIE (Personal Information Removed)
