I purchased a Ninja Kitchen System advertised on TV on November 9, 2013 by phone. At that time, I agreed to the offer from Member Rewards: a 30-day free trial, a $20 rebate, and a free $50 Walmart gift card.
*My Ninja Kitchen System arrived over two weeks later. I went through the printed materials and there was nothing about Member Rewards, nor my $50 gift card. So I kept an eye out for anything in the mail from Member Rewards.
*My December bank statement had a charge of 24.95 from Member Rewards. I had not received anything from Member Rewards yet. So I searched through old/junk mail and found something from "FREE Rewards Plus." This apparently was the Member Rewards information - but under a different name! It was postmarked November 20, 2013 and received the last week of November - about three weeks from the date of my purchase. Yet the date on the "card member" was November 9, 2013. Not only was the company name different from the original name provided, but worse - this was the first communication and yet the 30-day free trial was already more than half over! *
So I read through the mailer and found in paragraph 10: "FREE Gift Card and Cash Back Reward - To receive the Gift Card and Cash Back Reward we promised you, log on to www.freerewardsplus.com and click on "Forms" in the main navigation bar to download your redemption forms."
I followed these instructions and there is no form!
I called the provided number and there was only a recording. No options. Not even an opportunity to leave a message.

And far more than worse, the $20 rebate was only good for 30 days from the activation. In other words, I received membership info about three weeks after the "activation date" and under a different company name. the convoluted process required that I fill out yet another form and so there was no chance that I'd be able to get the $20 REBATE. Nor the promised $50 Walmart card. And now i have also been charged $24.95 - twice! THIS IS BEYOND REPROACH!
$24.95x2 plus $50 plus $20 = $119.90
