Sixteen charges to my bank account for $18.20 ranging from March 2012 to June 2013. I have sent an email to Mutual of Omaha (because a google search of the charges description indicates this is a Mutual of Omaha company - for lack of a better word) but have not received a response yet. This charge went unnoticed due to the number of overwhelming life events over the past 18 months experienced by me and my husband.

A rep from Mutual of Omaha claims I made the purchase of 2 polices for 2 of my children in 12/2011 over the phone. I never purchase any type of insurance over the phone.

Tracey Hall, FLMI, HIAA, ACS, states the policies were purchased over the phone on 12/07/2011. I requested a recording of said conversation.

My response verbatim: "I have never purchased ANY insurance over the phone. Do you have a recording of this alleged conversation? I want a refund in full."

Now a the rep claims the policies were purchased online. I have requested copies of the policies as well as copies with full details of the original policy application.
