Case History



On or about May 8, 2013, I ordered an item from Costco Mail Order. Shortly after starting the order,
A page comes up to do a survey. Only about 3 questions. By doing the survey you could get a choice of
A gift from their list and only pay for the shipping. I did this 2 times giving my Visa card number and
Address. A few days later I received both items. Charges on my Visa was only for shipping. I sincerely
thought I was doing a survey for Costco..

At month end I received my Visa Statement. These 2 companies had billed me for their products which
I had not authorized.

I called Card Services and talked to a female, Angela. And described my problem. She pulled up a current
List of items for June. There was another charge from one of the companies. This will show on my June statement. At this time I had our cards cancelled. It could have been a lot worse.

Companies and list of items:

BFM Miumilano: 5/9/12 fgt. charges $5.99 Legit
5/23/13 charges $98.00 Unauthorized product (never Received)

SW Webb Vaporfume 5/9/13 fgt. Charges 4.95 Legit
5/21/13 Charges $109.67 Unauthorized product (never received)
6/11/13 Charges 69.62 Unauthorized product (not received)

I checked both of these companies on the Internet and they had a very long bad history of problems such
as I mentioned above.

This is the first time I have encountered this problem. My husband and I are senior citizens living on a fixed income. This sort of fraud is uncalled for.

Please remove these items from our account. Thank you for the excellent service you have given us.
