My name is Diane (Personal Information Removed)& am writing on behalf of Dr. Ann (Personal Information Removed)of Thunderbird Family M.D. Her employee Starrlene (Personal Information Removed)signed Dr. Caywood on above website. Ms. Fetzer paid for this with her own account at Banner Federal Credit Union. She did not advise Dr. Caywood. Months later Dr. Caywood got billed from the company. A collector Gary Stone began harassing her on a daily basis. I spoke w/his supervisor on 4/12/13 Ken Ryder. Balance was $2600 & he would settle for $1500.I called Dr. Caywood & she said she had no intentions of paying. Advised Mr. Ryder, this # is 877-524-5478 ext 231. The date I listed is approximate. Ms. Fetzer is no longer employed. Her address is (Personal Information Removed) Phone #'s are (Personal Information Removed) &(Personal Information Removed). We Have set up a phone recorder both at my home & Dr. (Personal Information Removed)office, as Mr. Stone is extremely rude & cruel.
