I ordered Pure Green Coffee as advertised by Dr. Oz. I was promised a $60.00 gift certificate to Walmart. Upon receiving the merchandise there was no invoice nor any gift certificate. Attempts to call Qwest Laboratories, 888-497-6012 were met with a recording "application error occurred" I have no idea what that means. Anyway I would like to redeive a refund and ensure I do no receive any more merchandise.
