I did not get approved, so I was happy to see that there was a guarantee. I followed the instruction and there was fax number given in order for me to send over the 5 denial letters. I reached out to them via email you would think that they would have included this information, WRONG! I finally get a contact number to call in and their answering service is as generic as they come, very unhelpful, just leave your message and all will be well, NOT! Also this information can not be found anywhere on their website under their "guarantee", under the Q%A section, no where!

Hi there, if any or your other clients should need the fax number to ULSC it is: 347-713-7751. The company did respond in a timely manner and I received a reply the next business day.

As of today I have yet to receive my refund of $34.95. I sent a 5 denial letters via fax on 1/23/13. I've reached out to them again via email.
