I requested a "free" bottle of Garcinia Cambogia on 11/18/2015 08:37am and a "free" bottle of Premium Cleanse on 11/18/2015 08:38am. The agreement was to pay $4.95 shipping on each bottle.
My November bank statement shows the following charges:
11/19/2015 1118LFN*LIFENUTRA 855-2052376 UT 2454395A2S66 $4.95
11/20/2015 1118HEALTHLIFE8777 877-7884673 UT 2425477A3DSL $1.99 (for recycled packaging I didn't request)
11/20/2015 1118BODYHEALTH8778 877-8882340 UT 2425477A3DSL $4.87
The order did not say there would be recurring charges, only that it was a limited time offer (not a "trial" offer). I received both bottles packaged in a single box on 11/23/2015. There was no paperwork included an no contact information. Only the last 2 digit of the 4 digit street address are clear (xx14)
Today, this charge was posted to my bank account:

I did NOT authorize this additional charge.
