Filled out application at http://www.cosmeticdentistrygrants.org/CosmeticDentistryGrants/application_enroll.action/ I was told that a specialist had been assigned my case and would contact me by phone within 48 hours, they never did. I was then sent an email saying my application was denied and to write to them to get specifics. Seeing how I make $638 dollars a month how could I not qualify? Turns out they always have large up front fees for X-rays and "prep" work they know they can not get this from me so I was denied. Here is their address as well, dont fall for their scam.
