She took my information and said she will forward it to the correct supervisor and they will call back within 24-48 hours. I attempted to contact Cyvita, LLC at 9:49am by dialing (800) 968-1387 and spoke with a representative. I informed her of the profile and of our efforts to contact somebody with the authority to look into this profile. She stated that I would have to send information by mail or by fax.

I informed her of the profile and asked to speak to the appropriate channel in regards to this. She stated that the only way to get a response for these matters are to send a request via fax.

I called Cyvita at 800-968-1387 on 10/19/2012. I spoke to a Customer Service Rep named Sammy. I alerted him to the pubic profile that is on Scambook. I asked him for corporate contact information for Cyvita and he said that they didn't have any and the only way to contact someone appropriate would be to send a fax. I gave him my information to pass off to his Supervisor. I await their response.

I contacted Cyvita at 11:19am PST by dialing (800) 968-1387 and spoke with Sammy in customer service. Sammy informed me that he was instructed by his supervisor, Mr. Gunther to have me send a letter to P.O. Box 9107 Pueblo, CO 81008 USA regarding this matter. He told me I could also send a fax to (719) 948-1910.
